About the role

ADEYSONS HEALTH LLC is hiring a certified MOBILE Community Health Worker (CHW) with a focus on MENTAL HEALTH outreach to support our Houston community. This is a mobile, field-based role — you travel to residents at their homes, community sites, and events (not an in-office position), connecting people to mental health care, resources, and follow-up support. Schedule & terms: • Part-time: 24 hours/week (8 hours/day, 3 days/week) • 4–6 month W-2 contract position • Competitive pay: $20.00/hour • Mobile / field role across Greater Houston (reliable vehicle required) What you'll do: • Conduct mental health outreach and education in the community • Connect residents to mental and behavioral health services and provide warm referrals • Complete follow-up contacts and check-ins to keep clients engaged in care • Help residents navigate resources, appointments, and support programs • Coordinate directly and professionally with our clients throughout your shift • Provide clear, accurate progress reports at scheduled team meetings • Report to our facility as needed during your scheduled shift hours • Represent our clients and ADEYSONS HEALTH professionally at all times • Perform other related duties as assigned by your supervisor ADEYSONS HEALTH is an Equal Opportunity Employer. Requirements

Active Texas DSHS Community Health Worker (CHW) Certification
Interest or experience in mental / behavioral health outreach
Valid Texas Driver's License and reliable vehicle for mobile/field work
BLS / HIPAA / OSHA certification — or obtain within 30 days

Nice to have

Prior mental health, community outreach, or public-health experience
Bilingual (English/Spanish) a plus
