The bridge between you and your physicians — appointment reminders, referrals, lab follow-up, and medication reconciliation for patients managing multiple conditions.

Frequently asked.
Is a Community Health Worker the same as a doctor or nurse?+
No. A CHW is a trained frontline public-health worker who helps you navigate, access, and stay engaged with the healthcare system. We do not diagnose, prescribe, or provide clinical treatment — but we work closely with your existing providers to make sure their plan actually works for you.
Do you take insurance?+
Texas has limited Medicaid billing for CHW services (mostly restricted to the CPW — Case Management for Pregnant Women & Children — provider pathway). Most clients pay out-of-pocket, through an employer wellness benefit, or through a physician-clinic contract. We offer sliding-scale rates for uninsured / underinsured Houstonians.
What languages do you serve, and what communities?+
English and Yoruba. We specifically serve Houston's Yoruba, Nigerian, and broader African-immigrant community with culturally-grounded coaching, workshops, and materials — while remaining open to any Houston resident who wants our support.
Where do sessions happen?+
Your choice — in your home, at our Houston center on Bissonnet, at your workplace or church, or by secure video/phone.
Can my clinic hire an embedded CHW?+
Yes. We contract with physician clinics, FQHCs, and small hospitals in Houston for embedded CHW panels — great for HEDIS quality metrics, no-show reduction, and Medicaid managed-care performance. Request a quote for custom scoping.
What's the DSHS certification status?+
Founder Adedeji A. Adeyanju is in active training with Texas DSHS certification expected December 2026. We disclose this transparently on every page. Group workshops and outreach are offered under Texas Ch. 146 pending final certification; formal CPW billing enrollment will follow after certification is issued.
