Hands-on enrollment in Medicaid, CHIP, and Marketplace plans. Benefits explained plainly. Provider matching, prior-auth support, and denial appeals.
Care Coordination
The bridge between you and your physicians — appointment reminders, referrals, lab follow-up, and medication reconciliation for patients managing multiple conditions.
Maternal & Child Health
Prenatal & postpartum coaching, WIC enrollment help, safe-sleep education, breastfeeding referrals, and postpartum-depression screening.
Community Workshops
60–90 min group education at your church, community center, employer, or clinic. Delivered in English or Yoruba. Diabetes prevention, heart health, senior wellness — culturally-grounded for Houston's African-immigrant community.
Health Fair & Outreach
Full-day presence at community health fairs, faith-based events, and school outreach — screenings, education, and warm-hand-off referrals.
Mental Health Outreach Specialist
Stigma-free mental-wellness support for Houston families, delivered by our dedicated Mental Health Outreach Specialist on the Community Health team. We meet you at home, on-site, or virtually — in English or Yorùbá.

Chronic-Disease Self-Management Coaching (1-hr session)
1:1 coaching for diabetes, hypertension, weight, medication adherence, and lifestyle change. In-home, at our Houston center, or virtual.
Insurance & Medicaid Navigation
Hands-on help enrolling in Medicaid/CHIP/Marketplace, understanding your benefits, finding in-network providers, and appealing denials. Per hour.
Care Coordination (per month)
Ongoing bridge between you and your physicians — appointment reminders, specialist referrals, follow-up on lab results, medication reconciliation. Best for patients managing multiple conditions.
Mental Health Outreach Visit
Stigma-free wellness check-ins with our Mental Health Outreach Specialist — supportive listening, mental-health education, screenings referral, and warm hand-offs to licensed counselors and psychiatric providers. Home, on-site, or virtual (per session). Not licensed clinical care.

Frequently asked.
Is a Community Health Worker the same as a doctor or nurse?+
No. A CHW is a trained frontline public-health worker who helps you navigate, access, and stay engaged with the healthcare system. We do not diagnose, prescribe, or provide clinical treatment — but we work closely with your existing providers to make sure their plan actually works for you.
Do you take insurance?+
Texas has limited Medicaid billing for CHW services (mostly restricted to the CPW — Case Management for Pregnant Women & Children — provider pathway). Most clients pay out-of-pocket, through an employer wellness benefit, or through a physician-clinic contract. We offer sliding-scale rates for uninsured / underinsured Houstonians.
What languages do you serve, and what communities?+
English and Yoruba. We specifically serve Houston's Yoruba, Nigerian, and broader African-immigrant community with culturally-grounded coaching, workshops, and materials — while remaining open to any Houston resident who wants our support.
Where do sessions happen?+
Your choice — in your home, at our Houston center on Bissonnet, at your workplace or church, or by secure video/phone.
Can my clinic hire an embedded CHW?+
Yes. We contract with physician clinics, FQHCs, and small hospitals in Houston for embedded CHW panels — great for HEDIS quality metrics, no-show reduction, and Medicaid managed-care performance. Request a quote for custom scoping.
What's the DSHS certification status?+
Yes. CHW services are delivered by our hired Texas DSHS-certified Community Health Worker staff, including a dedicated Mental Health Outreach Specialist. All coaching and outreach are peer education & navigation under Texas Health & Safety Code Ch. 146.
